Practice Policies

Practice Policies

Our policies on eligibility, fees, payment, cancellations, prescribing, and communication.

Eligibility & Telehealth

We see adults 18 years of age and older. All appointments are conducted by telehealth. You must be physically located in Tennessee or Virginia at the time of your visit.

Medicare & Medicaid

We are unable to see patients who have any Medicare coverage (including Medicare Advantage) or Medicaid coverage (including TennCare and Virginia Medicaid).

If you will be traveling outside Tennessee or Virginia, please contact us at least 24 hours in advance to reschedule. Visits joined from outside these states are treated as missed appointments and the full visit fee is charged.

Fees, Insurance & Payment

New patient visit

60 minutes

$350

Follow-up visit

20 minutes

$175

Payment is handled separately from the online appointment request, according to our financial policy. If you do not have an active card on file, you will receive a secure email link to add one when your appointment is created. Adding a card does not itself charge it.

We are in the process of credentialing with insurance plans. At this time, appointments remain self-pay. Please contact our office to confirm whether we participate in your specific plan before scheduling. We can provide a superbill — an itemized receipt — that you may submit to your insurance company for potential out-of-network reimbursement. We cannot guarantee what your insurer will cover.

You have the right to receive a Good Faith Estimate of expected charges. Learn more

Cancellations, Late Arrivals & No-Shows

Please notify us as early as possible if you need to cancel or reschedule.

  • Cancellations with less than 24 hours' notice are charged the full visit fee.
  • If you do not join your telehealth visit within 10 minutes of the start time, the visit is considered missed and the full visit fee is charged.
  • If you join your visit from outside Tennessee or Virginia, the visit is considered missed and the full visit fee is charged. Please notify us at least 24 hours in advance if you will be traveling so we can reschedule.
  • There is no charge if a practice-side technology failure prevents the visit from being completed.
  • Any refund owed under our policy will be issued within 45 calendar days after it becomes due, or sooner if applicable law requires.

Appointment Requests

New-patient appointment requests require staff review and approval before they are confirmed. Existing patients can book without staff approval. Staff aim to respond within one business day. Please rely on your appointment confirmation, not an acknowledgment of your request. Lacey Nicolau is not yet booking appointments.

Prescribing & Monitoring

Medication treatment is individualized after a clinical evaluation. When clinically appropriate and legally permitted, treatment may include controlled medications such as stimulants, benzodiazepines, sedatives or sedative-hypnotics, and weight-management medicines such as phentermine. Prescribing is at the clinician's discretion and is never guaranteed.

Patients receiving controlled medications must complete a controlled-medication treatment agreement and follow the recommended visit and safety-monitoring plan. In-person evaluation or additional testing may be required. Telehealth alone may not always be sufficient.

Communication & Privacy

Messaging is not a substitute for a medical appointment. Please schedule a visit to discuss new or worsening symptoms, diagnoses, or changes to your treatment or medications. Do not use messaging for urgent medical concerns. Holston Health Associates does not provide emergency care. If you are experiencing a medical or mental health emergency, call 911 or go to the nearest emergency department. Do not wait for a message response. Do not send medical details by regular text, regular email, or website forms.
